PIP form help — we fill in the PIP2 form for you
The PIP2 form is long and asks about how your disability affects your daily life. We translate the questions into your language, you give us the answers, and we type them into the official form.
Personal Independence Payment (PIP) helps with extra living costs if you have a long-term physical or mental health condition or disability. The claim has two stages: a phone call, then a form called 'How your disability affects you' (the PIP2 form).
The PIP2 form is where most people need help. It asks detailed questions about daily living and mobility. You must return it within 1 month, and what you write matters for your claim.
What the PIP claim involves
- The claim starts by phone on 0800 917 2222 (Monday–Friday, 8am–5pm), or by posting a letter to Freepost DWP PIP 1.
- After that you get the PIP2 form — online or by post. It has sections on your conditions, medication, daily living and mobility.
- You might also be asked to attend an assessment if more information is needed.
Official process: GOV.UK: PIP — How to claim.
How the PIP2 form works
- Start the claim: You call the PIP new claims line or write to Freepost DWP PIP 1. You'll be asked for your National Insurance number, bank details, doctor's details, and dates for any hospital or care home stays.
- You receive the PIP2 form: The form 'How your disability affects you' arrives online or by post within about 2 weeks. You have 1 month to return it.
- You tell us your answers: We translate each question into your language — Urdu, Punjabi, Polish, Romanian, English and more. You answer in your own words, by voice, text or video call.
- We complete the official form: We type your answers into the PIP2 form in English and include the supporting documents you have (prescription lists, care plans, letters from your doctor or care team).
- You review and submit: We return the completed form and a translation of your answers. You check it, sign it and send it. If DWP needs an assessment, that's arranged separately.
